The Opportunity


We are seeking Physician Assistants (PA-C) and Nurse Practitioners (NP) to join our growing team across post-acute care settings. In this role, you will:


  • Manage medically complex, high-acuity patients across SNF and LTAC settings
  • Conduct admissions, rounding, and ongoing follow-up visits
  • Partner with physicians, PAs and NPs to deliver team-based, coordinated care
  • Ensure CMS compliance while maintaining a patient-first approach
  • Build meaningful relationships with patients, families, and facility partners
  • Contribute to quality improvement and value-based care initiatives

Qualifications

  • Licensed Physician Assistant (PA-C) or Nurse Practitioner (NP) - Massachusetts license or eligible
  • Experience in SNF, LTAC, or hospital medicine preferred
  • Strong clinical and documentation skills
