The Home Health Registered Nurse (RN) is responsible for delivering high-quality, patient-centered care in the home setting. This role involves performing comprehensive assessments, developing and managing individualized care plans, and coordinating with an interdisciplinary team to ensure optimal patient outcomes. The RN will utilize OASIS documentation, provide education to patients and families, and monitor patient progress while working independently in the field.
