Manage a consistent caseload of patients in their homes
Perform comprehensive assessments and develop individualized care plans
Provide hands-on skilled nursing care and patient education
Monitor progress and adjust care plans to ensure the best outcomes
Collaborate with physicians, therapists, and the broader care team
Build meaningful relationships with patients and families during critical moments in their healthcare journey What You'll Need
Active, unrestricted RN license
At least 1 year of nursing experience (home health experience is a plus, not a must)
Strong clinical assessment and critical thinking skills
A valid driver's license, reliable transportation, and auto insurance
Willingness to travel within your assigned territory
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
