Join a Home Health team in Urbandale, ID as a Physical Therapist providing in-home evaluations and treatment for patients across the lifespan. Plan, implement, and evaluate individualized plans of care based on physician referral and ongoing assessment. Collaborate with an interdisciplinary team to support patient function, safety, and independence. Perform patient and family education, recommend and adapt durable medical equipment, and document care in compliance with clinical and regulatory requirements. This role requires mobility for home visits and the ability to perform physical tasks associated with patient care.


Minimum Qualifications

  • Current State Physical Therapist license valid in Idaho required.
  • Bachelor’s degree in Physical Therapy from an APTA-approved program required.
  • Proof of current CPR certification required.
  • Current driver’s license, reliable transportation, and vehicle insurance required.
  • 1–2 years of experience as a Physical Therapist in acute care or rehabilitation required.
  • Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking required.
  • Ability to work a flexible schedule and travel locally required.
  • Must read, write, and speak fluent English.
  • Must meet mandatory continuing education requirements.


Preferred Qualifications

  • Previous home health or home care experience preferred.
  • Bachelor of Science in Physical Therapy noted; advanced degrees may be considered preferred.


Assignment Details

  • Works in patients’ homes within the local service area.
  • Performs home visits and may be required to adapt equipment and intervention plans to home environments.
  • Must be able to travel locally between patient visits.

Impacting Patient Care

Deliver patient-centered rehabilitation in the home setting to restore or maintain function. Complete clinical assessments, develop and revise Plans of Care, document progress and collaborate with physicians and the interdisciplinary team. Instruct patients and families on safe transfer techniques and proper use of assistive devices. Apply infection control and universal precautions. Supervise other team members when applicable and accept assignments consistent with professional competence.
