We invite you to be part of our team, delivering primary medical management in partnership with our respected PACE Organization (PO) Physicians. As a full-time Nurse Practitioner, you will perform assessments, create care plans, and guarantee that participants receive thorough care while nurturing a compassionate and supportive atmosphere.

Key Responsibilities:
  • Work alongside PO physicians to conduct assessments and execute care plans.
  • Engage in discharge planning and provide treatment for acute illnesses.
  • Make home visits as required.
  • Contribute to the development of clinical policies and procedures.
  • Participate in the provider on-call rotation.
Essential Qualifications:
  • Master’s degree in Nursing.
  • 5-7 years of experience, ideally in long-term care settings.
  • Current license to practice in North Carolina.
  • CDS Certification, DEA Registration, and Board Certification are mandatory.
  • Valid BLS certification.
  • Willingness to travel within the designated geographic area.
