  • Additional Information: Provide skilled interventions focusing on mobility training, gait training, balance retraining, transfer training, and functional restoration. Develop and implement individualized treatment plans and collaborate closely with physicians, nursing staff, occupational therapists, speech-language pathologists, and case management teams. Complete documentation in accordance with hospital standards, and educate patients and caregivers on treatment plans, safety strategies, and discharge recommendations.
