What You ll Do:
- Conduct screening, evaluation, and rehabilitation of patients with physical functioning disorders
- Develop and implement individualized care plans following all regulatory standards
- Administer procedures including manual therapy, therapeutic exercises, modalities, wound care, and device support
- Monitor patient progress and adjust treatments accordingly
- Collaborate with healthcare team members for holistic patient care
- Document all interventions and patient outcomes thoroughly in electronic records
- Support and supervise rehabilitation aides or students when needed
- Participate in care conferences, education sessions, and training activities
What You Need:
- Valid Oklahoma license or eligibility to practice
- BLS certification
- Proven experience in long-term or rehabilitation settings
- Strong clinical assessment skills and patient-focused approach
- Ability to work independently and as part of a multidisciplinary team
- Excellent communication skills
