Equis Care Psychological Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time capacity. This full-time position allows for remote work, enabling you to maintain a work-life balance while engaging with patients effectively. You will primarily conduct assessments and meet with clients virtually, utilizing telehealth platforms and electronic health records to manage documentation and communications seamlessly. The setting fosters flexibility while ensuring high standards of care and support for our clients.

• Conduct comprehensive psychosocial assessments through interviews, observations, and by reviewing medical records, to evaluate and understand the emotional, social, and environmental factors affecting each patient’s recovery pathway.


• Provide individual and family counseling, focusing on adjustment issues related to recovery, emotional resilience, and coping strategies tailored to the specific circumstances of each patient.


• Collaborate closely with our interdisciplinary rehabilitation team to develop and review individualized treatment plans that address psychosocial needs, with measurable goals and outcomes.


• Educate and support patients in accessing essential community resources such as vocational rehabilitation services, housing assistance, or support groups, facilitating their successful reintegration into society post-rehabilitation.


• Participate in discharge planning activities, ensuring a smooth transition for patients returning home or moving to alternative care settings, providing necessary referrals and follow-up resources.

Our client population includes a diverse range of individuals facing challenges associated with substance use disorders, co-occurring mental health conditions, and physical disabilities. You will interact with clients who may be experiencing significant emotional distress, adjustment difficulties, and want to reintegrate into their communities effectively. Your role will be critical in understanding their unique experiences and guiding them through their rehabilitation journey.

• A Master’s degree in Social Work from an accredited institution is required.


• Active state licensure as a Licensed Clinical Social Worker (LCSW).


• Proven experience in counseling and social work practices, particularly within rehabilitation or medical settings.

• Experience working in substance use treatment or within a detox center environment.


• Familiarity with community resource networks and post-rehabilitation support services.


• Strong background in facilitating group therapy sessions or support groups.


• Excellent organizational skills with a focus on patient-centered care and an ability to coordinate with various stakeholders effectively.

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience, plus a sign-on bonus.
