The Home Care Social Worker will be an integral part of our Detox Center's home care agency team, collaborating with nurses, caregivers, and healthcare professionals to implement client-centered strategies effectively. • Perform comprehensive assessments of clients' psychosocial needs, including emotional, mental, and social factors.


• Develop personalized care plans collaboratively with clients and their families, aligning with their unique goals and preferences.


• Provide emotional support and counseling to seniors and their families, guiding them through transitions such as aging, health changes, or loss.


• Advocate for clients' rights, ensuring they have access to necessary community resources and services.


• Collaborate closely with multidisciplinary teams, ensuring seamless coordination between various care providers to enhance client care.


• Conduct regular home visits to assess clients’ living conditions, monitor their progress, and address any emerging safety or wellness concerns.


• Facilitate family communication and conflict resolution, aiming for harmonious collaborative care.


• Educate clients and their families on available community resources, support groups, and programs to support social engagement.

The Home Care Social Worker will primarily serve seniors transitioning into or out of home care. • Bachelor's degree in Social Work from an accredited institution is required.

The salary for this position ranges from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
