Work EnvironmentThis full-time position allows for flexibility as it is entirely remote, offering the opportunity to work from the comfort of your home while effectively collaborating with team members through digital communication platforms. As you support patients, you will engage in virtual therapy sessions, manage casework, and participate in team meetings to discuss patient progress and collaborate on treatment planning.
Key Responsibilities- Patient Safety and Care Management: Ensure that discharge appointments are confirmed and all necessary arrangements are made with patients and their loved ones prior to discharge.
- Communication and Coordination: Maintain open lines of communication with referral sources and outpatient providers to ensure continuity of care from admission through to discharge and follow-up.
- Crisis Management: Address and manage crises that arise during therapy sessions and within the treatment milieu, using appropriate interventions to maintain a supportive therapeutic environment.
- Group Facilitation: Lead psychoeducational and therapeutic groups, providing a safe space for patients to express their emotions and learn coping strategies.
- Clinical Evaluation: Conduct assessments of patients to develop individualized treatment plans while regularly updating care strategies based on patient needs.
- Patient Transfer Coordination: Facilitate and oversee transfers to higher levels of care for patients who require additional support or intervention.
- Clinical Outreach: Actively participate in outreach activities including program tours to raise awareness and understanding of services offered to potential clients and their families.
You will primarily work with patients in hospice care, focusing on those with terminal illnesses and their families. This role requires sensitivity, empathy, and experience in navigating complex emotional landscapes. You will assist patients in managing feelings of grief, anxiety, and depression while empowering them to maintain dignity and autonomy during their final stages of life.
Qualifications- A Master’s degree in Social Work from an accredited program.
- Active and unrestricted license as a Clinical Social Worker (LCSW) in the state of California.
- Excellent verbal and written communication skills, with the ability to effectively document and report on patient progress.
- Demonstrated ability to establish and maintain therapeutic boundaries with clients.
- Capability to navigate and utilize digital platforms for patient documentation and communication purposes.
- Previous experience in hospice, palliative care, or a similar healthcare environment is highly desirable.
- Experience leading group therapy sessions and conducting psychoeducational classes.
- Familiarity with electronic medical record (EMR) systems and telehealth platforms.
The salary for this position ranges from USD 120,000 to 130,000 per year, commensurate with your experience and caseload.
