• Conduct comprehensive psychosocial evaluations of seniors to determine their emotional, mental, and social needs, documenting findings accurately.


• Collaborate closely with clients and their families to craft tailored care plans that reflect their aspirations and needs, adjusting these plans as necessary.


• Provide counseling and emotional support, helping clients navigate transitions such as relocation, health changes, or loss.


• Advocate for clients' well-being, ensuring their access to vital community resources, health services, and support networks.


• Engage with interdisciplinary teams, including healthcare providers and community services, to enhance the care coordination process.


• Monitor clients through regular visits, assessing their home environments, and addressing any emerging safety or health-related concerns.


• Facilitate communication and resolve conflicts among family members involved in the care process to ensure harmony and effective care implementation.


• Educate clients and caregivers about available community support, programs, and resources that promote social integration.

Your primary focus will be on senior clients facing various psychosocial challenges, including mental health issues, isolation, and the impacts of aging. • A Master’s degree in Social Work from an accredited institution.


• Current and valid licensure as a Master Social Worker (LMSW) in Texas.


• A minimum of two years of related experience in social work, preferably in a community health or geriatric care setting.
