Clinical Social Worker (LICSW) for Mental Health Services in Hospice Care

This position is entirely remote, allowing you the flexibility to provide high-quality care from your own workspace while remaining deeply engaged with our hospice team. You will interact regularly with multidisciplinary healthcare professionals, including nurses, doctors, and spiritual care providers, using telehealth solutions to conduct assessments and therapy programs. • Conduct in-depth mental health assessments for hospice patients, gathering comprehensive information about their life history, presenting issues, mental well-being, and coping strategies.


• Collaborate closely with patients and their families to develop and implement personalized treatment plans that address mental health challenges and promote emotional resilience.


• Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), trauma-focused therapy, and supportive counseling, tailored to the unique needs of each patient.


• Facilitate individual and group therapy sessions aimed at fostering emotional stability and providing support through various end-of-life challenges.


• Work alongside other healthcare professionals to coordinate care, providing a holistic approach that integrates physical, spiritual, and emotional health.


• Manage crisis situations by providing immediate support, utilizing de-escalation techniques, and developing action plans for higher-level care when necessary.


• Deliver psychoeducation to patients and families, helping them understand the psychological aspects of the dying process and available mental health resources.


• Maintain meticulous documentation of clinical assessments, treatment plans, session notes, and correspondence in adherence to legal and ethical standards.


• Engage in multidisciplinary team meetings to enhance collaborative care strategies and participate in ongoing staff training initiatives to broaden clinical knowledge.

Your primary focus will be hospice patients, many of whom may be experiencing complex emotional responses related to grief, anxiety, and existential concerns. You will also have an opportunity to support their families, aiding them in coping with the emotional challenges associated with terminal illness and loss. This role is pivotal in ensuring quality emotional care during a sensitive time, which can often correlate with enhanced patient outcomes and family satisfaction.

• A Master’s Degree in Social Work from an accredited institution is required.


• Active and valid LICSW licensure in the state of Washington.


• A minimum of two years of post-licensure experience in mental health practice, ideally within hospice or end-of-life care settings.


• Thorough understanding of mental health disorders and expertise in trauma-informed care approaches.


• Strong ability to perform mental health evaluations, formulate diagnoses, and craft effective treatment strategies.


• Experience leading both one-on-one and group therapy sessions employing a variety of therapeutic techniques.


• Excellent crisis management skills, with composure and efficacy in high-pressure scenarios.


• Exceptional interpersonal communication skills, enabling connection and understanding with diverse populations.

• Experience working in hospice or palliative care settings is highly preferred.


• Familiarity with telehealth platforms and remote therapy practices, providing flexibility in client interactions.

The salary range for this position is $100,000 to $120,000 annually, based on your experience and qualifications.
