Mindful Steps Counseling Services is actively seeking a Licensed Master Social Worker (LMSW) dedicated to providing empathetic support for seniors in Fort Lauderdale, Florida. Work EnvironmentThis role supports a remote work setting, allowing for the flexibility to engage with clients in their homes or various community locations. While the position is primarily remote, regular communication and collaboration with an interdisciplinary team—including nurses, caregivers, and healthcare providers—are essential to ensure comprehensive care. In this role, you will also have scheduled visits to clients, balancing flexibility with consistency to meet their ongoing needs.
Key Responsibilities- Conduct thorough psychosocial assessments of senior clients, incorporating emotional, mental, and social dimensions.
- Collaborate with seniors and their families to create personalized care plans that align with their goals and preferences.
- Provide counseling and emotional support, assisting clients in navigating life transitions such as health changes and bereavement.
- Advocate for client rights and ensure access to community resources, guiding clients toward appropriate services.
- Engage in regular client monitoring through home visits, evaluating their living conditions and addressing safety concerns.
- Facilitate communication among family members to resolve any conflicts and promote teamwork in care efforts.
- Educate clients and families about available community resources, fostering social engagement and improving quality of life.
- Maintain accurate documentation of assessments, care plans, progress notes, and relevant communications, utilizing electronic health record (EHR) systems efficiently.
- Stay informed about developments in social work practices and regulations to maintain ethical standards.
The focus of this role is working with senior clients, who may face a variety of challenges associated with aging, including chronic illnesses, mental health issues, and social isolation. Providing tailored support to this population requires an understanding of aging-related issues and the ability to empathize with clients and their families as they navigate these complex transitions.
Qualifications- A Master's degree in Social Work (MSW) from an accredited institution is required.
- Current and valid state licensure as a Licensed Master Social Worker (LMSW).
- A minimum of two years of experience in social work, preferably within community centers or geriatric settings.
- Strong knowledge of social work principles, community resources, and aging-related challenges.
- Exceptional assessment, counseling, and problem-solving skills with a compassionate approach.
- Demonstrated ability to establish rapport and communicate effectively with seniors and their support systems.
- Proficient in using electronic health record (EHR) systems and standard office applications.
- Experience working in hospice or palliative care settings.

- Demonstrated ability to work within interdisciplinary teams to manage client care effectively.
