Key Responsibilities- Conduct thorough clinical evaluations and assessments to identify patient needs and formulate appropriate treatment plans.
- Lead evidence-based therapy sessions and facilitate psychoeducational groups aimed at enhancing coping skills and emotional resilience.
- Assess and monitor patients for suicidality and threats to self or others, implementing crisis management strategies when necessary.
- Ensure appointment confirmations are communicated clearly to patients and their support systems prior to discharge.
- Interact with referral sources and outpatient providers at admission, midpoint, and discharge to ensure continuity of care.
- Facilitate patient transfers to higher levels of care when clinical escalation is needed.
- Support milieu staff in their interactions with patients and provide guidance on clinical best practices.
- Actively participate in clinical outreach and program tours, promoting our services to potential patients and stakeholders.
- Complete all required documentation, including treatment plans and clinical notes, accurately and promptly.
- Collaborate effectively with the treatment team and engage in case management tasks to drive comprehensive patient care.
Your primary focus will be on a diverse client base ranging from adolescents to adults, each facing unique mental health challenges. Our patients often present with complex diagnoses, including depression, anxiety, PTSD, and other co-occurring disorders. You will be expected to tailor your therapeutic approaches to meet the individual needs of each patient, promoting personalized care and recovery.
Qualifications- Currently hold an independent license as a Licensed Clinical Social Worker (LCSW).
- Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.
- Strong proficiency in electronic health record (EHR) systems and Microsoft Office 365 applications.
- Demonstrated ability to maintain professional boundaries and exhibit effective communication skills.
- Experience in documentation practices including initial assessments, SMART treatment plans, and clinical case notes.
- Experience working in diverse settings, particularly in community health or non-profit organizations.
- Familiarity with providing therapeutic services in a telehealth environment.
- Ability to thrive in a collaborative team setting and manage multiple tasks efficiently.
- Competitive salary between $120,000 to $130,000 annually for full-time employment.
