Position Overview

Heluna Psychotherapy Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in providing exceptional care to patients in a hospice setting. Key Responsibilities
  • Conduct thorough psychosocial assessments by engaging with patients through interviews and observations, as well as reviewing medical records to identify emotional, social, and environmental factors that may affect their rehabilitation journey.
  • Provide compassionate counseling and support for patients and their families, assisting them in navigating the emotional landscape of grief, adjustment issues, and coping with their conditions.
  • Collaborate closely with rehabilitation professionals—including physical therapists, occupational therapists, and medical staff—to create and implement individualized treatment plans tailored to each patient’s specific psychosocial needs.
  • Assist patients in identifying and accessing available community resources, such as support groups, vocational rehabilitation services, housing assistance, and other post-rehabilitation services.
  • Engage in discharge planning, facilitating smooth transitions for patients returning to their homes or moving to other care settings, ensuring continuity of care.
  • Maintain accurate documentation in compliance with HIPAA regulations and utilize electronic health record systems to track progress and treatment outcomes.
Patient Population / Client Focus

In this role, you will primarily work with patients facing life-limiting illnesses, disabilities, and complex rehabilitation needs within a hospice context. Our patient population varies widely and may include individuals recovering from strokes, orthopedic surgeries, or neurological disorders. By providing tailored support and resources, you will contribute significantly to the enhancement of their quality of life during a critical time.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW) in California.
  • Proficiency in using electronic health records and telehealth platforms effectively.
  • Strong communication and interpersonal skills that foster trust and rapport with patients and their families.
Preferred Experience
  • Previous experience in a rehabilitation, hospice, or medical care setting.
  • Knowledge of psychosocial dynamics, adjustment issues, grief counseling techniques, and community resource navigation.

Compensation and Benefits
