Work EnvironmentThis position is fully remote, allowing you to provide care from a comfortable setting while maintaining flexibility in your work schedule. You will engage with an interdisciplinary team that may include physicians, nurses, and other social work professionals, ensuring that our commitment to high-quality patient care is met. Scheduled shifts will vary, incorporating days, evenings, and weekends based on departmental needs, with on-call responsibilities as necessary.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial challenges, tailoring interventions to meet individual needs.
- Provide one-on-one and group therapy sessions aimed at addressing issues such as grief, adjustment to illness, and coping strategies.
- Collaborate with healthcare teams to create and implement comprehensive care plans that include psychosocial elements necessary for patient welfare.
- Coordinate discharge planning by connecting patients with community resources to ensure a smooth transition and continuity of support following hospice care.
- Remain actively engaged in crisis intervention, providing immediate support to patients and families facing acute emotional distress.
- Compile and maintain meticulous documentation of patient interactions and care plans, ensuring alignment with legal and organizational standards.
- Participate in multidisciplinary team meetings and hospital committees, contributing your expertise in social work to enhance clinical operations and patient care strategies.
Your primary focus will be on patients receiving end-of-life care. This population requires not only medical support but also profound emotional and psychological assistance as they navigate the complexities of their conditions. Working with patients and families in these sensitive situations, your role will be crucial in fostering an environment of understanding, compassion, and cultural sensitivity.
Qualifications- A Master’s degree in Social Work from an accredited institution is required.
- Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
- Demonstrated knowledge of medical terminology, mental health disorders, and effective crisis intervention techniques.
- Exceptional communication and interpersonal skills, with the ability to build rapport with diverse populations.
- A minimum of two years of clinical experience in healthcare, particularly in hospice or palliative care settings, is preferred.
- Experience working with interdisciplinary teams and familiarity with documentation software relevant to patient care.
