Clinical Social Worker - Hospice Specialist

Astor Youth & Families Services is seeking dedicated and compassionate Licensed Clinical Social Workers (LCSWs) to join our team as full-time professionals at our Hospice Center. This role necessitates an experienced social worker who specializes in providing psychotherapy and support to individuals facing end-of-life challenges. This position is primarily remote, designed to provide flexibility while supporting the needs of our hospice clientele. • Conduct comprehensive clinical assessments, utilizing best practices to formulate individualized treatment plans for clients in hospice care.


• Administer targeted therapeutic interventions based on evidence-based practices, adapting techniques to meet each client's distinct needs.


• Maintain meticulous clinical documentation, ensuring all records comply with organizational and regulatory standards.


• Collaborate with a multidisciplinary team, including medical professionals and other therapists, to ensure holistic care for clients.


• Participate in case consultations and peer review meetings to continually enhance the quality of care provided.


• Empower clients and their families through education, equipping them with essential mental health knowledge and support tools.


• Engage in ongoing professional development by remaining current with research and practices in social work and hospice care.

This role focuses on clients in a hospice setting, a unique and sensitive area within the mental health and medical fields. You will support individuals dealing with terminal illnesses, providing not only therapeutic interventions but also emotional support to their families. Understanding the complexities of grief, bereavement, and end-of-life issues will be crucial in making a meaningful impact.

• A Master’s degree in Social Work from an accredited institution.


• Valid License as a Clinical Social Worker (LCSW) in the state of Texas.


• Minimum of three years of clinical experience with a focus on psychotherapy, preferably in hospice or palliative care settings.


• Strong skills in solution-based psychotherapy interventions and other therapeutic modalities.


• Exceptional communication and interpersonal skills, fostering positive relationships with clients and colleagues.

• Experience working with bereaved families and understanding the nuances of grief therapy.


• Familiarity with multidisciplinary team dynamics and care coordination.


• Knowledge of relevant regulations, ethical standards, and documentation practices in social work.
