Mindful Steps Counseling Services is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in Arlington, Texas. This full-time, remote position offers a unique opportunity to provide vital support to seniors within our hospice care setting. As a Senior Licensed Social Worker, you will play a crucial role in assessing and addressing the psychosocial needs of elderly clients, helping to enhance their quality of life during a sensitive time.

Work EnvironmentWorking remotely from your home office, you will be an integral part of our hospice team, collaborating with healthcare professionals, caregivers, and family members to deliver coordinated care. The nature of this role requires flexibility, as you may need to adjust your schedule for client visits and on-call support. Typical work hours are Monday through Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs.
Key Responsibilities- Conduct comprehensive assessments of seniors' emotional, mental, and social needs, identifying key factors that impact their overall well-being.
- Develop tailored care plans in collaboration with clients and their families, ensuring alignment with their goals and preferences.
- Provide counseling services to both seniors and their families, offering emotional support during significant life transitions and challenges.
- Advocate for clients by facilitating access to essential community resources and ensuring their rights are upheld.
- Collaborate with interdisciplinary teams, including medical professionals, to create coordinated care strategies that address comprehensive wellness.
- Regularly visit clients to monitor their status and safety, providing timely interventions as necessary.
- Educate clients and families on available programs and resources that enhance social engagement and quality of life.
- Maintain accurate and thorough documentation of client interactions, assessments, and progress using electronic health record systems.
- Stay current with best practices in social work, adhering to ethical standards and regulations to guide your practice.
Your primary focus will be on supporting elderly clients in a hospice care setting. This unique population often faces a spectrum of emotional and social challenges, including coping with grief, managing transitions, and navigating complex family dynamics. Qualifications- Master's degree in Social Work (MSW) from an accredited institution.
- Current and valid state licensure as a Licensed Master Social Worker (LMSW).
- A minimum of two years of relevant social work experience, preferably in a hospice, geriatric, or community service setting.
- In-depth knowledge of social work principles, aging issues, and available community resources.
- Strong assessment, counseling, and crisis intervention skills.
- Exceptional communication and interpersonal skills to foster collaboration with clients and interdisciplinary teams.
- Proficiency in electronic health record (EHR) systems and other digital tools to ensure efficient documentation and communication.
- Experience working specifically within hospice or palliative care environments.
- Familiarity with end-of-life issues and the psychological complexities associated with them.
- Demonstrated ability to engage effectively with families, including conflict resolution skills and mediation techniques.
