Clinical Social Worker - Hospice Care Specialist

We are seeking a dedicated Clinical Social Worker to join Henriquez Mental Health Clinic, focusing on providing critical mental health support to patients in a hospice care setting. This role will primarily be conducted remotely, allowing for flexible work arrangements while maintaining effective communication with our multidisciplinary team. As a part of our hospice center, you will collaborate with healthcare professionals, including doctors, nurses, and other specialists, ensuring that your assessment and interventions align with the overall treatment goals for each patient. You will have access to virtual meeting tools, electronic health records systems, and other digital resources necessary for patient management.

• Conduct thorough evaluations and mental health assessments tailored to hospice patients, considering their specific needs and circumstances.


• Implement effective behavior management strategies to address the unique challenges faced by individuals in hospice care.


• Provide individual and family counseling, utilizing evidence-based psychotherapy techniques to foster emotional healing and coping skills.


• Collaborate closely with medical staff, including physicians and nurses, to develop and monitor care plans that address both physical and emotional well-being.


• Empower families by providing educational resources, strategies, and emotional support to navigate the complexities of end-of-life care.


• Maintain comprehensive and up-to-date clinical documentation in the electronic health record systems to ensure continuity of care.


• Participate in regular case management meetings to discuss patient progress and adjust treatment approaches as needed.

This role focuses on a diverse client population within a hospice environment. You will work primarily with terminally ill individuals and their families, addressing complex emotional and psychological needs, and facilitating discussions around end-of-life decisions. Your expertise will be crucial in helping clients cope with grief, loss, and the transitions associated with hospice care.

• A Master’s degree in Social Work or a related field from an accredited institution.


• An active LCSW (Licensed Clinical Social Worker) license in California.


• Minimum of 2 years of direct clinical experience, particularly in mental health services or hospice care.


• Proficiency in using MS Outlook, Word, and electronic health record management software.


• Strong documentation skills with an understanding of HIPAA regulations regarding patient confidentiality.

• Experience working in hospice or palliative care settings is highly advantageous.


• Familiarity with therapeutic modalities tailored for trauma, grief, and hardship.


• Understanding of community resources and support systems available to hospice patients and their families.

The Clinical Social Worker position offers a competitive annual salary ranging from $110,000 to $130,000.
