Futuristic Wellness Sanctuary is seeking a dedicated and compassionate Clinical Social Worker for a full-time position as a Licensed Independent Clinical Social Worker (LICSW). This vital role entails working remotely, primarily focusing on hospice care, where you will support patients and their families through end-of-life planning and psychosocial support.

Work Environment

This position offers a remote work setting, allowing for flexibility while ensuring team members can collaborate effectively. Our team relies on various telehealth tools and electronic medical records systems to manage cases efficiently. You will participate in regular virtual meetings to discuss patient progress and share insights with your colleagues.

Key Responsibilities
  • Collaborate closely with interdisciplinary healthcare teams to create, implement, and adjust treatment and discharge plans informed by psychosocial assessments and evidence-based social work interventions.
  • Conduct comprehensive assessments for patients experiencing emotional or behavioral challenges, designing individualized interventions aimed at improving their mental health and overall well-being.
  • Document all clinical interactions, including progress notes and assessment findings, in compliance with department protocols to ensure continuity of care and to facilitate successful patient outcomes.
  • Offer counseling and crisis intervention services tailored to patients and families, assisting them in navigating complex emotions related to terminal illness, grief, and loss.
  • Engage with patients' families and support systems to coordinate care, fostering understanding and facilitating effective discharge planning.
Patient Population / Client Focus

Your role will focus on serving patients within a hospice care context, which includes working with individuals facing terminal illnesses and their families. You will provide emotional support and end-of-life care planning, ensuring that patients receive the compassionate support necessary during one of life’s most challenging transitions.

Qualifications
  • A Bachelor’s degree in Social Work or a related field from an accredited institution is required.
  • Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State is mandatory.
  • Demonstrated proficiency with computer systems, including word processing software, spreasheet tools, email platforms, and electronic medical records.
  • Exceptional communication skills and the ability to work collaboratively in a fast-paced, dynamic healthcare environment.
Preferred Experience
  • A minimum of 2-3 years of professional experience in social work, ideally within a healthcare or hospice setting.
  • Experience working directly with terminally ill patients and their families is highly desired.
  • Expertise in crisis intervention and supporting emotional and psychological needs in sensitive situations will be evaluated favorably.
Compensation and Benefits

This position offers a competitive salary in the range of USD 100,000 to 105,000 annually, along with a comprehensive benefits package that includes:
