Your expertise will contribute significantly to the overall well-being and comfort of our clients and their families.

About the Organization

At Gotham Enterprises Ltd, we are committed to providing exceptional end-of-life care through our Hospice Center located in West Hartford, Connecticut. Our organization prioritizes a holistic approach to care, recognizing the importance of addressing not just physical but also emotional and psychological needs. We value a supportive and collaborative environment where our team members can thrive and make a meaningful impact in the lives of our patients and their families.

Work Environment

This role is fully remote, allowing you to perform your duties from the comfort and safety of your home. We provide you with the necessary tools and technology to connect with clients, including secure video conferencing applications for virtual counseling sessions. Work hours are flexible, enabling you to create a schedule that prioritizes both client needs and your personal lifestyle.

Key Responsibilities

As a Remote Hospice Social Worker, your primary responsibilities will include:

  • Conducting individualized assessments to understand the unique needs and preferences of hospice patients and their families.

  • Utilizing video chat technology to provide ethical and compassionate counseling sessions focused on emotional support and coping strategies.

  • Regularly assessing the progress of clients, making necessary adjustments to treatment plans to enhance effectiveness.

  • Collaborating with interdisciplinary teams, including nurses, doctors, and other healthcare professionals, to create a comprehensive support network for patients.

  • Educating clients and their families about available resources, support services, and coping mechanisms to manage grief and loss.

Patient Population / Client Focus

You will work primarily with stable, low-acuity adults who are receiving hospice care, along with their families. Your role will be critical in providing emotional support and facilitating open communication about end-of-life decisions. Empathy and sensitivity to the emotional challenges faced by clients will be essential in your approach.

Qualifications

The ideal candidate for this position will possess the following qualifications:

  • A Bachelor's degree in Social Work, Psychology, or a related field is required.

  • A current and unrestricted Licensed Clinical Social Worker (LCSW) license is mandatory.

  • Ability to work independently with minimal supervision, demonstrating accountability and professional judgment.

  • Strong interpersonal skills and a sympathetic approach in dealing with sensitive and emotionally charged matters.

  • Proficiency with computer systems and stable Wi-Fi connectivity to conduct online sessions efficiently.

Preferred Experience

While not mandatory, preference will be given to candidates with:

  • Experience in hospice, palliative care, or working with terminally ill patients.

  • Demonstrated success in developing and implementing treatment plans.

  • Familiarity with telehealth practices and online counseling methodologies.

Compensation and Benefits

The annual salary for this role ranges from USD 90,000 to 104,000, depending on experience and hours worked. In addition to competitive pay, we offer:

  • A sign-on bonus to support your transition into our team.

  • Weekly direct deposit for your convenience.

  • A monthly healthcare stipend for full-time counselors to help cover medical expenses.

  • Minimal paperwork burden, allowing you to focus on patient care rather than administrative duties.
