Work Environment

This full-time role is primarily onsite, where you will conduct house calls, allowing you to work in a comfortable and familiar environment for your patients. Your typical work schedule will consist of 36 to 40 hours per week, providing a structured yet flexible approach to your shifts, fostering both professional growth and work-life balance. As a member of our team, you will collaborate with fellow Nurse Practitioners, physicians, and healthcare professionals to ensure seamless patient care and enhance the patient journey.

Key Responsibilities

• Conduct thorough assessments of patients in their homes, gathering comprehensive medical histories and performing physical exams to diagnose both acute and chronic health conditions.
• Develop, implement, and refine individualized, evidence-based treatment plans in collaboration with other healthcare providers, ensuring a team-oriented approach to patient care.
• Order and interpret diagnostic tests, prescribing appropriate medications, therapies, and follow-up interventions tailored to the needs of each patient.
• Educate patients and their families on disease management strategies, medication adherence, and self-care practices to empower them in their healthcare journey.
• Actively participate in clinical discussions and interdisciplinary rounds, sharing insights and updates on patient progress with the team to ensure optimal outcomes.
• Maintain accurate, detailed, and timely electronic medical records within our healthcare informatics system, ensuring compliance with regulatory standards.
• Lead and contribute to quality improvement initiatives and actively engage in continuing professional development to maintain current knowledge in the ever-evolving field of healthcare.

Patient Population / Client Focus

Your work will predominantly involve caring for individual adult patients with varied health conditions, including chronic diseases such as diabetes and hypertension, as well as acute cases requiring immediate attention. You will have the opportunity to form meaningful relationships with your patients, focusing on their unique health needs and helping them navigate their healthcare in a convenient and accessible manner.

Qualifications

• Valid certification as a Nurse Practitioner and an active nursing license in the state of Texas.
• A Bachelor's degree in Nursing or a related field from an accredited institution.
• A minimum of three years of clinical experience in a relevant healthcare setting.
• Excellent communication and collaborative skills, with an ability to work effectively within a multidisciplinary team.
• Strong critical thinking and problem-solving abilities, enabling you to manage complex patient scenarios and develop innovative care solutions.

Preferred Experience

• Experience in concierge medicine or home health care is highly desirable.
• Prior involvement in quality improvement projects and familiarity with electronic health record systems is a plus.

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary range of USD 120,000 to 220,000 per year, based on your experience and qualifications.
