Key Responsibilities

• Assess patients, review medical histories, and evaluate their healthcare needs.


• Develop and manage individualized treatment plans.


• Monitor patient progress and coordinate follow-up care.


• Collaborate with physicians and other healthcare professionals to support effective patient outcomes.


• Maintain accurate medical records and follow clinical and compliance standards.


• Educate patients about their conditions, treatments, and ongoing care.

Requirements

• Active Nurse Practitioner license in New York State.


• At least 1 year of Nurse Practitioner experience.


• Strong clinical assessment, communication, and decision-making skills.


• Experience in vascular medicine, cardiology, emergency medicine (ER), critical care, or internal medicine is preferred.
