Work Model: Hybrid - field visits in Westchester County + 4 days remote work

Hybrid role: Field + 4 days Remote

Field work: 6 home visits completed in 1 day (Brooklyn territory)

Remote work: Care coordination, documentation, and follow-ups

Training location: Brooklyn office

Orientation: Bronx

About the Role

We are seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines limited field work with remote care coordination, offering strong work-life balance while making a meaningful impact in the community.

What You'll Do

• Conduct in-home assessments to evaluate members' clinical, functional, and social needs
• Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams
• Coordinate care across home care, hospital, rehabilitation, and long-term care settings
• Complete approximately 6 home visits per week within Brooklyn
• Monitor patient progress and adjust care plans to ensure optimal outcomes
• Support safe discharge planning and reduce hospital readmissions
• Communicate with members, families, caregivers, and providers
• Maintain accurate and timely documentation in compliance with guidelines

Qualifications

• Active, unrestricted RN license in New York
• Associate's degree in nursing required (BSN preferred)
• Minimum 3 years of RN experience
• Experience in MLTC, home care, geriatrics, or case management preferred
• Strong communication, organizational, and critical thinking skills
• Ability to work independently in a hybrid environment
• Bilingual skills are a plus
