Join a integrative non-profit integrated regional health system in NW Ohio (outside of Toledo). Suburban community offering a high quality of life, affordable housing and good school districts. Expanding inpatient Palliative Care inpatient service. Hospital-employed position. Support 6 patients/day. Monday - Friday (8am-4:30pm/5pm). Focus on symptom management, care coordination, transitions of care, and ensuring treatment decisions align with each patient's goals and values. Assist with discharge planning, including transitions to home-based palliative care or hospice. Shared evening and weekend rotation (only phone consults).
